Government approves mechanism for mixed financing of innovative medicines procurement

The Cabinet of Ministers of Ukraine has amended Resolution No. 61 of 27 January 2021, which defines the procedure for conducting negotiations, concluding, and executing managed access agreements. The amendments introduce a new approach to the procurement of innovative medicines through managed entry agreements (MEAs) under a mixed financing model.

From now on, such procurements can be carried out simultaneously using funds from the state budget, local budgets, and healthcare facilities.

“Thanks to the managed entry agreement mechanism, we have the opportunity to procure innovative and high-cost medicines on significantly more favourable terms than would be possible on the open market. This not only saves money but also ensures that patients with severe and rare diseases receive medicines that were previously effectively unavailable. Overall, in 2025, new agreements were concluded for the supply of seven modern medicines for such patients,” emphasised Viktor Liashko, Minister of Health of Ukraine.

Previously, procurements of innovative medicines were carried out centrally using only state budget funds. Now, both regional health departments and individual state and communal medical facilities will be able to join the process. Procurement participants will conclude relevant agreements with the Medical Procurement of Ukraine (either when conducting new negotiations with suppliers or for additional procurements under already concluded agreements, subject to agreement on possible volumes and prices). In turn, this will enable better price offers, increased procurement volumes, and optimised spending on medical needs.

In addition, the mixed financing model will open new opportunities for healthcare facilities, as it will allow them to procure innovative medicines that were previously inaccessible due to high cost, and to apply treatments in line with modern international clinical protocols, thereby ensuring patients’ access to effective treatment and providing healthcare facilities with new opportunities for development and improvement of medical service quality.

Participation by facilities in such procurements is voluntary. At the same time, funds from the Programme of Medical Guarantees, which are received through the National Health Service of Ukraine, cannot be used for this purpose (these funds are intended exclusively for financing the guaranteed package of medical services, which will continue to be provided separately under the Programme of Medical Guarantees).

Background

The Medical Procurement of Ukraine has been using the Managed Entry Agreements mechanism since 2022 for procuring high-cost innovative medicines.

This mechanism allows innovative medicines to be purchased at fair prices acceptable to the state budget. Thanks to confidential arrangements with manufacturers, the state can provide patients with the most modern treatments much more quickly. In many cases, the price of a medicine can be reduced several times over.

The Ministry of Health continues to work on expanding patients’ access to modern treatment methods. Ensuring effective and free medical care for patients remains one of the key priorities of state policy in the healthcare sector.
